In public health, timing matters. Clear communication matters. Trust matters.
For large county health agencies serving millions of residents, the ability to deliver accurate, secure, and timely outreach can directly impact community health outcomes. When a nationally accredited public health authority serving more than 2.6 million residents sought to modernize its communication infrastructure, the goal was not simply to upgrade technology.
The goal was to better protect and serve its community.
When Manual Outreach Limits Public Health Impact
Prior to modernization, the agency relied on GovDelivery and manual communication processes to support vaccination reminders, appointment confirmations, and public health updates. Outreach efforts were often department-specific and siloed, making coordinated campaigns difficult.
Key challenges included:
- Manual follow-ups for vaccination second doses
- Limited cross-channel communication capabilities
- Difficulty segmenting audiences based on CRM data
- Compliance and security concerns when communicating health information
- High staff effort required for ongoing outreach
As vaccination campaigns expanded and health programs evolved, these manual processes became increasingly unsustainable.
Public health communication needed to become smarter, faster, and more secure.
Our Strategy: A Connected and Secure Citizen Engagement Ecosystem
Over an 11-week implementation, we partnered with the agency to design and deploy a modern Salesforce Marketing Cloud environment fully integrated with Salesforce CRM.
This was not simply a platform setup. It was a shift toward next-generation public health engagement.
We began with discovery sessions to align use cases around vaccination follow-ups, appointment reminders, missing documentation, and secure communication workflows. From there, we configured a scalable foundation designed for both immediate needs and future growth.
Core components included:
- Sender Authentication Package and secure email infrastructure
- Business Unit architecture and governance setup
- Reply Mail Management and sender profiles
- CRM integration for real-time data synchronization
- Preference center development
- Sandbox configuration for safe testing and iteration
- Ad Studio activation for social retargeting
- SMS short code acquisition and configuration
By intentionally architecting the platform from the ground up, the agency gained a secure, compliant, and scalable communication environment.
Intelligent Vaccination Journeys
One of the most impactful implementations focused on vaccination follow-ups.
Using CRM data, individuals due for a second Mpox dose automatically entered a coordinated journey. Residents first received an email reminder. Engagement behavior then guided the next step.
Those who opened the message progressed through a tailored communication path. Those who did not engage were retargeted through social media advertising and received follow-up email reminders. If there was still no engagement, SMS outreach provided an additional touchpoint.
This multi-channel orchestration reduced manual staff follow-up while increasing the likelihood that residents completed their vaccination schedule.
No spreadsheets. No one-off sends. Just coordinated, data-driven outreach designed to protect community health.
Secure Communication for Sensitive Health Information
Public health agencies must balance proactive outreach with strict compliance requirements.
To address concerns around sharing personal health information, we designed a custom double-authentication workflow. Before sensitive details were sent via email, recipients were required to verify specific identifying information through a secure form.
Only when the submitted data matched CRM records would the system trigger the appropriate communication. If verification failed, no health information was delivered.
This ensured that PHI remained protected while allowing the agency to communicate efficiently and responsibly.
Security was not an afterthought. It was embedded into the architecture.
Expanding to Multi-Channel Public Health Engagement
For the first time, the agency could coordinate communications across:
Email for program outreach and reminders
SMS for urgent or time-sensitive follow-ups
Paid social retargeting to re-engage non-responsive audiences
Integrated CRM data enabled improved segmentation and more personalized messaging across programs, from disease prevention campaigns to social services initiatives.
Outreach became connected instead of siloed.
Enablement That Sustains Innovation
Technology transformation only succeeds when teams feel confident using it.
Throughout the engagement, we delivered comprehensive training and knowledge transfer sessions, equipping both marketing and technical staff to manage journeys, segmentation, and cross-channel campaigns independently.
The implementation transitioned into managed services support, ensuring continued optimization as public health needs evolve.
The result was not just a new platform, but a sustainable communication model built for long-term impact.
The Result: Smarter, Safer Public Health Communication
Within 11 weeks, the agency achieved:
- Reduced manual staff outreach
- Automated vaccination and appointment follow-ups
- Improved segmentation powered by CRM data
- Secure handling of sensitive health information
- Multi-channel engagement across email, SMS, and social
- A scalable foundation for future public health initiatives
Most importantly, residents now receive timely, coordinated communication that supports better health outcomes.
Why This Matters
Public health agencies operate at the front lines of community well-being. When communication systems are fragmented, opportunities to protect and serve are missed.
When engagement is secure, connected, and data-driven, agencies can respond faster, reach further, and build stronger trust with the communities they serve.
At JourneyBlazers, we help public sector organizations modernize responsibly. From architecture to activation, we design secure engagement ecosystems that support both compliance and community impact.
Because in public health, communication is more than outreach.
It is protection!
